Select the Diagnostic order form if a simple test socket is being ordered without liners. This way customer service will have all the necessary information for the order. See below:

Please only use the Össur Portal - TT check socket option if the Össur Portal is being used to send information about socket (amputation level, plastic, alignment, suspension) Because this information is captured in the Össur Portal, we've reduced the fields that need to be completed in Design Studio. However, if there isn't an order in the Portal, all this information will be missing.

If the order form above is selected, customer service will match the Patient ID and PO number to the associated Portal order with the same ID and PO. See the associated Portal Order below:
